 Advanced Directives
Living Will/Durable Power of Attorney
When you were admitted, you were asked if you had completed these documents. Every patient in Ohio has the right to make these decisions.
The living will allows you to decide the medical care you receive if you become terminally ill or permanently unconscious and are unable to make your wishes known.
The durable power of attorney for health care allows you to name a person to act as your representative in making health care decisions for you if you become unable to make them for yourself. It is very important that we have a copy of this document to fully honor your advanced directives wishes. Please provide these documents as soon as you are reasonably able to make arrangements to have them available.
A living will or durable power of attorney form can be completed at any time during your stay. Contact the pastoral care department at 7-2584 if you wish to complete these forms.
Do Not Resuscitate (DNR) Order
State regulations offer a Do Not Resuscitate (DNR) Comfort Care and Comfort Care Arrest Protocol as developed by the Ohio Department of Health. A DNR Order means a directive issued by a physician or under certain circumstances, a certified nurse practitioner or clinical specialist, that identifies a person and specifies that cardiopulmonary resuscitation (as defined by law for this purpose) should not be administered.
The DNR Comfort Care and Comfort Care Arrest Protocol list the specific circumstances and actions that paramedics, emergency medical technicians, physicians or nurses will take when attending to a DNR Comfort Care or Comfort Care Arrest patient. The protocol also lists what specific actions will be implemented.
You may talk to your doctor about DNR Comfort Care and Comfort Care Arrest Order and Protocol options.
So that we may better meet your wishes for care, please provide a copy of the Living Will, Durable Power of Attorney and/or Do Not Resuscitate Order if these documents have been completed by the patient prior to admission.
